Clinical Application of Multiple Modalities
While you have the option to use all components at once, the choice depends on the client's tolerance and the specific goals of the desensitization phase.
Increasing Cognitive Load (EMDR 2.0)
Combining visual, auditory, and tactile stimulation is a highly effective way to increase cognitive load. Following EMDR 2.0 principles, if a client's processing stalls or they habituate quickly to a single stimulus, adding pulsators and headphones maximizes working memory taxation. This forces the brain to work harder to track multiple synchronized stimuli, which can help restart stalled processing or deepen desensitization for highly charged memories.
When to Use a Single Modality
Standard EMDR protocols typically begin with visual stimulation. You might choose to stay with one modality for clients who:
Are easily overstimulated or prone to high levels of dissociation.
Have specific sensory sensitivities (e.g., omitting pulsators for a client who dislikes tactile input).
Demonstrate sufficient working memory taxation with eye movements alone.

Best Practices for Therapists

If you find that the "emotional charge" of a memory is not decreasing, or if the client is not sufficiently engaged by the task, gradually introduce the Pulsators or Headphones to increase the working memory demand.
